DOCTOR APPOINTMENTS

Three visits within one week

A package of three round trips within one week for visits to one destination on an agreed schedule. Contact HIND for a subscription quote.

DIALYSIS APPOINTMENTS

Three visits per week, booked monthly

A monthly transportation arrangement for dialysis visits three times per week. Round trips are arranged to one destination on an agreed schedule.

Important subscription details

A trip means an outbound and return leg: a round trip.
Each round trip includes up to 5 miles each way (10 miles total).
Monthly subscriptions cover every scheduled visit in the calendar month, including extra scheduled days in longer months.
Waiting time is not included.
Subscriptions are for one destination on a fixed schedule; schedule changes are not included.
Unused trips are not refundable. Subscription cancellations do not incur an additional cancellation fee.
Service area 2 and 3 subscriptions require operations approval.

Subscriptions must be arranged and purchased through HIND staff. The standard ride portal does not sell subscriptions.
